Per Diem Hospice Nurse Practitioner Job Location

Overview The Nurse Practitioner provides quality hospice and palliative care to assigned patients across the life span (perinatal, pediatrics, and adults) while consulting with the team physician, associate medical director and chief medical officer (CMO). Consults, assesses, and evaluates the plan of care while collaborating with the IDT (interdisciplinary team), patient, family, and community physicians. Responsible for carrying out the mission, vision, values, and commitment to quality of The Elizabeth Hospice. The Elizabeth Hospice is San Diego's oldest non-profit hospice of 45 years. We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego county and south Riverside county with multiple hospice office locations. Our Mission: To enhance the quality of life for those nearing the end of life's journey and for those who grieve. Salary: $74 - $82 hourly. Subject to experience and qualifications. Work Schedule: Varies, 8-hour shifts. Candidate must provide availability for 6 shifts per month, 2 of which must be a weekend day. Work Location: San Diego County, CA Travel: Travel is required for patient visits. Responsibilities 1. Responsible for visiting patients within a short period of time following admission and asmedically necessary afterwards. Qualifications 1.
