Job Summary: This Stroke Program Coordinator position supports the stroke program at United Hospital, in collaboration with the stroke medical director and the interdisciplinary team. This position is integral to maintaining stroke certification at United Hospital. United Hospital is a comprehensive stroke center, certified by DNV. This position includes but is not limited to maintaining DNV certification, completing continuous survey readiness, survey follow-up, clinical program development, quality management, community outreach, patient safety, staff education, and being and providing expert knowledge in nursing care of stroke patients.

Job Description: Leads and coordinates clinical practice for a population, a department or community. Ensures that quality, education and patient care clinical practice are assessed, implemented and evaluated.
Principle Responsibilities
- Coordinates clinical practice for the department or community.
- Ensures that quality, education, and patient care clinical practice is assessed, implemented, and evaluated.
- Improves patient outcomes by applying improvement methodology to quality initiatives.
- Ensures that clinical practice is consistent with care delivery model and professional standards.
- Reviews patient records on a concurrent basis to identify opportunities to improve care.
- Monitors and addresses issues of safety and risk.
- May provide ongoing assessment of the patient and collaborate to advance the plan of care.
- May coordinate discharge and post-hospitalization needs for patients.
- May participate in the development and implementation of clinical and patient standards and pathways.
- May do consultation with other disciplines, and coordinate multidisciplinary activities to maximize patient outcomes and satisfaction.
- Facilitates strategic, quality and operational planning for a department, program or population.
- Supports organizational performance improvement.
- Collaborates to develop and implement department plans.
- Provides leadership for compliance with regulatory standards, professional practice guidelines and standards.
- Leads the implementation of protocols and guidelines.
- Coordinates development and revision of policies and procedures.
- Coordinates and evaluates safety and emergency preparedness plans.
- Participates in councils and committees in a collaborative governance structure.
- Facilitates gathering and analysis of data and information for identification of issues and evaluation of progress.
- Supports performance improvement teams by developing project plans, supporting improvement trials, and monitoring critical success factors.
- May act as a resource in the utilization review process to optimize documentation.
- May work with external agencies and providers to enhance patient care.
- Provides leadership for orientation and staff development programs.
- Leads individuals and teams to design, implement, and evaluate orientation programs.
- Leads individuals and teams to design, implement, and evaluate staff development programs.
- Develops educational tools.
- Teaches/facilitates educational offerings for staff and providers.
- Coordinates learning experiences for students and other learners.
- Leads and coordinates teams for care improvement.
- Leads the integration of quality, education and practice goals and initiatives.
- May supervise individuals.
- May lead multidisciplinary conferences, case reviews, and presentations for care improvement.
- May participate in research activities for care improvement.
- Other duties as assigned.
Required Qualifications
- Bachelor's degree in Nursing, BAN or BSN
- 2 to 5 years in clinical experience, prefer critical care experience for most business units, or Rehab nursing experience for Rehab areas
- 2 to 5 years experience with electronic data capture and with Microsoft Word, Excel, PowerPoint and Outlook
- 2 to 5 years experience in process improvement

Licenses/Certifications
- Licensed Registered Nurse - MN Board of Nursing required for Minnesota practice settings upon hire
- Licensed Registered Nurse - WI Dept of Safety & Professional Services required for Wisconsin practice settings, for roles with direct patient care upon hire
- Certified Registered Nurse for specialty area within 6 months of hire
- BLS Tier 1 - Basic Life Support - Multisource required American Heart Association or Allina Health upon hire
- Certified Rehabilitation Registered Nurse For Rehab setting upon hire required

Pay Range
Pay Range: $45.40 to $62.82 per hour
The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
