Description

The incumbent in this position provides specialized patient care in the division of Gastroenterology in the Department of Pediatrics. This position will provide nursing duties as well as appropriate education to patients. In addition to providing specialized professional nursing services, the incumbent will assist with training related to their area of expertise in the education of other medical staff members, residents and medical students


Examples of Duties

Care Coordination and Direct Nursing Duties 90%

  • Demonstrates clinical competence in carrying out patient care duties specific to pediatric patients with gastrointestinal conditions
  • Determines patient/family eligibility for services/resources
  • Advocates for patient and family rights
  • Utilizes the nursing process for individuals and families who are patients of the Pediatric clinic. This includes telephone triage, direct patient care and patient/family education.
  • Interviews patients and families to determine dynamics and healthcare needs and identifies any contributing factors to illness; guides other staff members in patient teaching and encourages the patient's family to participate in teaching/care plan.
  • Screens telephone calls from patients, families, caregivers and others; answers questions using professional knowledge, expertise, nursing judgement; and evaluates the need for the patient to be seen
  • Coordinates follow up of hospitalized patients upon discharge, including homebound patients with patient/family, other health care providers, insurance companies, case managers, home care agencies, pharmacies, and primary care offices.
  • Coordinates follow-up of outpatient services. Provides follow-up care in conjunction with attending provider - providing information and teaching.
  • Demonstrates sound knowledge of diagnostic procedures utilized in the treatment and diagnosis of gastrointestinal disorders and provides education to patient/family
  • Designs an individualized plan of care with patient/family and foster a team approach by working collaboratively with providers and other members of the healthcare team.
  • Organizes and facilitates clinics
  • Assists providers with exams and/or procedures as needed
  • Communicates unmet patient/family goals and needs to responsible providers
  • Collaborates with the multi-disciplinary team and family to develop and participate in an individualized plan of care
  • Arranges and participates in care conferences as required.
  • Monitors and evaluates short and long-term patient responses to therapeutic interventions
  • Uses critical thinking skills to triage patient care priorities
  • Provides counseling and psychological support to families and patients as indicated by social and family circumstances
  • Coordinates care for new and established patients who require the initiation and/or continuation of medication infusions for the management of their medical condition.
  • Provides education to patients and families regarding gastrointestinal conditions, the provider's prescribed treatment and management plan, and lifestyle modifications
  • Coordinates/schedules periodic laboratory and radiologic testing
  • Assists families in coordination of insurance benefits and coordinates new referrals and return visits accordingly
  • Acts as a liaison between families and providers to facilitate communication.
  • Counsels family on the identified service needs and provides guidance as necessary
  • Other duties as assigned
Education and Research Duties 10%
  • Collaborates in the development of standards of nursing care related to gastrointestinal conditions
  • Guides other staff members in patient/family teaching
  • Attends in-services and other meetings as directed and deemed necessary; suggests ideas for in-service training and improving patient/family education
  • Collects and maintains current information regarding available community resources
  • Assists with the development and maintenance of educational materials for both patients/families and staff education
  • Participates in research and quality improvement measures
  • Assists faculty and/or residents in maintaining current case records and prepares reports as required.
  • Assists in maintaining and updating research database with appropriate and accurate information.

Qualifications

  1. Licensed as a registered professional nurse in the State of Illinois (If an Illinois Resident is licensed as a professional nurse in a state other than Illinois, the applicant must meet the criteria established by the Illinois Department of Financial and Professional Regulation to obtain the proper licensure within five (5) months of the date of appointment.)
  2. Bachelor's degree
  3. Two years of public health or specialized nursing experience
