GENERAL SUMMARY:

Responsible for identifying patient needs and priorities for care through assessment, planning for and delegating care based on patients needs, executing therapeutic physician-and nurse-initiated interventions, and evaluating the patient's response to care and progress toward outcomes. Whenever possible, includes the family or significant others in the plan of care of the patient. Coordinates care and communicates essential information between other professional disciplines as needed and uses available resources and innovative strategies to provide care. Empowers the patient and/or significant others to make informed changes in health related behaviors. Responsible for the promotion of primary, secondary and tertiary prevention.

PRINCIPAL JOB FUNCTIONS:

1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values.

2. *Initiates assessment based on the patient's presenting problems or needs and the physician's medical diagnosis by collecting key information from patient, family, and medical reports.

3. *Determines nursing diagnosis and priorities for care with special consideration given to patient teaching, goal setting, psychosocial well-being, and functional status.

4. *Assesses tolerance of activity levels, collaborates with other team members and makes recommendations/referrals regarding appropriateness of return to work or ADL's.

5. *Assesses and applies interpretation of diagnostic/monitoring data to the plan of care.

6. Formulates an appropriate exercise program including collecting and interpreting essential information from the participants chart (i.e. history, diagnostic test results, etc.); identifies participant musculoskeletal, metabolic and or risk factor limitations; applies graded exercise testing data, when available, to the formulation of the exercise prescription; obtains body compositional information; and formulates intensity, frequency, duration and progression of exercise prescription.

7. *Completes and updates individual treatment plans and ongoing assessments according to regulatory guidelines and GIRMC policy and/or as determined by patient condition. Continues to update nursing diagnoses/patient problems.

8. *Re-prioritizes care and sets new goals based on new information.

9. *Involves family and/or significant individuals in assessment activities.

10. *Documents appropriately in monitoring system and Electronic Medical Record (EMR) and is able to troubleshoot problems that arise.

11. *Appropriately interacts with patients/families from a variety of developmental and sociocultural backgrounds; seeks interpreter assistance as appropriate.

12. *Provides information to patients and families, reducing anxiety and conveying an attitude of acceptance, sensitivity and caring.

13. *Initiates communication with health care team members regarding treatment planning; informs appropriate health care team members (including physicians) of patient's condition/rehab status.

14. *Completes clinical/non-clinical assignments based on departmental workload and the qualifications and competencies of self and other staff members.

15. *Collaborates with staff and appropriate resource person(s) to formulate and update treatment plans and to work toward positive outcomes.

16. Initiates problem solving and conflict resolution skills to foster effective work relationships with health care team members and peers.

17. Assesses individual educational needs, barriers for learning, and learning styles and provides appropriate learning experiences. Reviews, develops and provides educational materials appropriate to patient needs.

18. *Provides education to patients and significant others regarding disease entities, risk factor management and behavior modification and promotes primary, secondary, and tertiary prevention in individual and group settings.

19. Supports and is involved in the hospital/departmental quality improvement initiatives; responds to problems/opportunities to improve care and/or processes.

20. Assists in employee, intern and student observation/orientation.

21. *Provides assistance in related program functions such as Pulmonary Rehabilitation and Enhanced External Counterpulsation.

22. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

23. Participates in meetings, committees and department projects as assigned.

24. Performs other related projects and duties as assigned.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

1. Maintains clinical competency as required for the unit including but not limited to age-specific competencies relative to patient's growth and developmental needs, annual skill competency verification and mandatory education and competencies.

2. Knowledge of hospital, staff and community based agencies and resources necessary to support the patient/family education.

3. Knowledge of federal and state regulations related to nursing applications and relative practice area.

4. Knowledge of computer hardware equipment and software applications relevant to work functions.

5. Ability to train, instruct and provide guidance to others.

6. Ability to work in a fast-paced environment related to changing patient needs including working with patients with acute, chronic and complex disease processes and those who are dying.

7. Ability to work with patients' growth and development needs, particularly related to patients who are undergoing lifestyle changes, adjusting to a chronic illness and/or pursuing health promotion prevention behaviors.

8. Ability to communicate effectively both verbally and in writing.

9. Ability to perform crucial conversations with desired outcomes.

10. Ability to establish and maintain effective working relationships with all levels of personnel, medical staff, volunteer and ancillary departments including diverse patient population.

11. Ability to problem solve and engage independent critical thinking skills.

12. Ability to maintain confidentiality relevant to sensitive information.

13. Ability to prioritize work demands and work with minimal supervision.

14. Ability to maintain regular and punctual attendance.

EDUCATION AND EXPERIENCE:

Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required. Minimum of one (1) year critical care experience required. Three (3) years of Cardiac Critical Care experience preferred.

OTHER CREDENTIALS / CERTIFICATIONS:

Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.

Advanced Cardiac Life Support (ACLS) certification required.
