Responsibilities

The Advanced Practice Clinician's primary role will be reviewing incoming clinical and medical considerations to ensure timely and appropriate placement within the hospital level of care. A secondary role is to assist in transitions of care and communication (warm-handoffs) from the hospital to adjacent programs and levels of care including residential, PHP, IOP, and or Outpatient Continuity Clinic providers. This person will work closely with interdisciplinary team members (i.e. Physicians, APCs, Nursing, Therapy, Admissions, receiving clinicians) to consistently and rapidly determine admission disposition and assist in smooth transitions of care.

  • Review clinical for safety concerns, including medical complexity.
  • Communicate and assist in defining persistent medical conditions (that may or may not be in our scope of capacity to safely manage within the hospital environment).
  • Interface with emergency department staff as needed.
  • Assess appropriateness for milieu.
  • Coordinate and facilitate interdisciplinary meetings as needed/directed.
  • Provide education to Patient Intake Coordinators as needed.
  • Track and analyze quality assurance measures.
  • Manage interdisciplinary conflict.
  • Communicate transitions of care to ensure smooth handoff of pertinent clinical information to receiving programs/settings (internal or external).

Qualifications

  • Requires Master's degree in Nursing or DNP or PA in Psychiatry.
  • Licensure as CNP with prescriptive authority required. Will also accept certified MN PA license.
  • DEA License Required.
  • Previous experience in an advanced psychiatric clinician role preferred.
  • Medical and psych triage experience required.
  • Emergency department experience preferred.
  • Computer skills with Microsoft Office Products including Outlook and Word required.

Pay range for this position is $110,000 to $140,000 depending on education and experience.
