Case Manager - Supervisor

Natchez Trace Youth Academy, a Universal Health Services, Inc. company, in partnership with KidLink Community Services, has a long history in providing comprehensive behavioral health care services to at-risk youth struggling with extensive mental health issues. Our facility is accredited by the Commission on Accreditation of Rehabilitation Facilities, which recognizes a commitment to measuring quality and standards of care. KidLink Community Services recruits foster parents and offers the training to be equipped to provide a safe, stable and therapeutic environment for children and youth that have been removed from their homes. Our training prepares staff and foster parents to support children and youth as they begin the healing process. Our therapeutic foster parents link children and youth to hope, healing and trust.

Natchez Trace Youth Academy is seeking a dedicated, compassionate Case Manager Supervisor to add to our growing team. This position works from home with a flexible schedule, with requirements to service the foster needs within the TN territory. This position oversees Middle TN Foster Care and Continuum Case Managers who coordinate the services needed for the child, family, foster parents, and other caregivers, working towards the goal of permanency. The Case Manager Supervisor is responsible for establishing and maintaining procedures for building effective relationships with families and agencies to provide the necessary services; and is responsible for overall direction, coordination, and evaluation of subordinate employees in accordance with the organization's policies and procedures and applicable laws.

Job Duties/Responsibilities:

  • Sets standards and expectations for case management paperwork quality, accuracy and timeliness that meets program guidelines
  • Applies disciplinary procedures as needed.
  • Monitor student movement and prepare written reports of projected discharges on a weekly basis.
  • Provide clinical consultation and reviews of admissions/referrals, as needed.
  • Provide direct training for orientation to new Case Manager staff. Facilitates staff development plans.
  • Ensures collaborative working relationships.
  • Ensures that all contractual regulations and requirements and all policies and procedures, regarding foster care/in home services, are being provided, Be knowledgeable regarding licensing and contractual requirements for all clients served, communicating information to other appropriate parties.
  • Review and approve all outside trainings/conferences of Case Manager staff
  • Review and approve all time off requests in Foster care/Continuum Care Departments.
  • Complete random chart audits to ensure quality documentation within the Foster Care/ Continuum Care Department and to ensure all charts are audit ready.
  • Coordinate and ensure Case Manager coverage of all clients during Case Manager short term or long-term absences.
  • Familiarizes self with the portion of DCS Provider Manual that applies to Foster Care/In Home Services and complies with changes as they occur.
  • Reviews ease files on a routine basis to ensure completed documentation is present for audits, and file reviews. Monitor documentation for appropriate and prompt tracking, objective, concise and legally defensible documentation charting, and reporting in software.
  • Aids in scheduling of meetings between all involved parties, community programs, medical appointments, to ensure quality care for clients.
  • Facilitates scheduling of Child and Family Team Meetings when necessary for program planning.
  • Completes required training sessions required in house and by outside agencies.
  • Ensures weekly, monthly, and quarterly recordings met and maintained.
  • Review and sign Monthly Progress Reports, IPP's, and other required documentation of clinical services provided by Case Managers.
  • Provide individual and family therapy for designated caseload and/or during absence of Case Managers.
  • Communicate with Director regarding case assignments at intake.

Qualifications

Requirements:

  • Master's degree in social work or human services field.
  • Case Management experience five (5) years with a minimum of three (3) years in a management role.
  • Valid state Driver's License with appropriate endorsement.
  • Automobile insurance with verification of medical liability.
