JOB DESCRIPTION:

SUMMARY OF POSITION

Engages with individuals to provide a safe and welcoming space that offers resources to enable the promotion of health. Works collaboratively with all programs internally as well as community partners to foster wrap around care for clients. Links individuals to appropriate services and resources as needed.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Remains up-to-date on resources in the community in order to be able to offer the most relevant information to consumers.
  • Coordinates between programs and staff to assist clients/potential clients to needed services
  • Provides ongoing education and support to clients/potential clients
  • Utilizes data tracking systems for documenting, monitoring and reporting program services, outcomes, etc for grant purposes
  • Engages in ongoing assessment of quality of services by coordinating satisfaction surveys and assisting in program improvement projects
  • Meets timelines for all service delivery, monitoring and reporting requirements for any semi-annual or annual grant reporting requirements. Report outcome measures as needed.
  • Attend staff meetings, and other internal meetings as assigned. Maintain appropriate communication with other employee and work as an effective team member.
  • Share personal recovery experiences and develop authentic, and appropriate, peer-to-peer relationships while providing recovery training and outreach to individuals in services as well as community members

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Knowledge of community resources.
  • Excellent customer service skills.
  • Skill in developing problem solving strategies
  • Knowledge of confidentiality guidelines

QUALIFICATIONS

Education Level

High School Diploma or equivalent

Years of Experience

Participation in own personal recovery process in order to care for self. Must be willing to share their recovery journey.
Experience providing behavioral health service preferred.

Must be 21 years of age

Certification/Licensure

CPR certification within 90 days of hire
Ability and willingness to obtain credentialing if required.

Pay Range:

$15.75--$22.00
