the lives of older adults through community, choice, and continuing care in the Clinical Educator & QA Nurse role. The Clinical Educator & Quality Assurance Nurse plays a critical role in ensuring high-quality patient care in home health and hospice services throughout Washington State . This position focuses on staff education, competency development, and quality assurance initiatives to ensure compliance with Washington State Department of Health (DOH), Medicare, Medicaid, and other regulatory agencies . You will provide continuing care through these responsibilities Essential functions of this position include the following. Clinical Education & Training: Develop and deliver Washington State-compliant training programs for new and existing staff, covering OASIS-E, palliative care best practices, Hospice Item Set (HIS), infection control, and state-specific regulatory updates . Provide hands-on clinical training, including end-of-life care, pain and symptom management, wound care, and advanced nursing interventions . Conduct ongoing competency evaluations, skills validations, and refresher courses to ensure staff proficiency in home health and hospice care . In partnership with Human Resources, develop annual skills competency program. Act as a resource for clinicians, offering guidance on documentation best practices and patient care in compliance with Washington State labor laws and nursing scope of practice . Educate staff on Washington State's Death with Dignity Act , advance directives, and hospice-specific legal requirements. Quality Assurance & Compliance: Monitor and audit patient records to ensure compliance with Medicare, Medicaid, and Washington State DOH regulations . Oversee OASIS review, ICD-10 coding accuracy, and regulatory documentation compliance for home health and hospice services. Develop quarterly QAPI performance improvement plans for home health and hospice. Conduct educational sessions with staff to provide trends and areas of focus for QAPI measures. Conduct clinical review of SOC, recertification and ROC visits. Assist in preparing for state and federal audits, accreditation surveys (ACHC, CHAP, or Joint Commission), and internal compliance reviews . Identify quality improvement opportunities and implement corrective action plans to enhance clinical performance, patient safety, and regulatory adherence . Lead investigations into patient complaints, adverse events, and infection control breaches , ensuring timely resolution and reporting per Washington State DOH guidelines . Through QA strategies and programs, ensure quality ratings, star ratings, and patient satisfaction scores exceed industry averages. Performance Improvement & Staff Support: Serve as a mentor and coach for clinical staff, promoting a culture of continuous learning, excellence, and compassionate patient care . Collaborate with leadership to develop evidence-based policies and procedures tailored to Washington State regulations . Track key performance indicators (KPIs) , such as hospitalization rates, patient satisfaction scores, and compliance trends , to drive quality initiatives. Lead interdisciplinary meetings focused on care coordination, regulatory updates, and performance improvement strategies . Ensure compliance with HIPAA guidelines and other company-related procedures for protecting confidential and sensitive information. Performs job in compliance with company policies, procedures and professional standards. Other duties and special projects as assigned. Our requirements and qualifications for success Bachelor's degree in Nursing or related field. Active Registered Nurse (RN) License in Washington State or out-of-state MSL. o Follow the Washington Nurse Practice Act. o If MSL, complete Suicide Prevention Training in advance of hire per state regulations. o If MSL, complete Washington State Board of Nursing (WABON) MSL demographic survey. Ability to obtain a CPR and First Aid certification within 30 days of hire. Effective and comprehendible English verbal and written communication skills to accomplish charting, record keeping, and to comprehend written orders and procedures. Minimum of two years' clinical nursing experience; home health experience preferred. Demonstrated knowledge of Performance Improvement and Quality Assurance programs. Knowledge of the regulatory requirements for home health and hospice, including the OASIS data collection and reporting requirements and OBQI expectations for Medicare Certified Agencies. Strong organizational and time management skills; ability to work with minimal supervision. Use logic and reasoning to identify alternative solutions, conclusions, or approaches to problems. Experience in research, analysis, and report preparation is desirable. Physical, environmental, and mental requirements Ability to stand and walk 2-3 hours at a time Ability to sit for periods of time for record review, phone consulting, and computer data entry. Ability to lift up to 40 lbs. Ability to listen to and comprehend information and ideas provided through conversation, email, and text. Salary Range: $41.98 to $60.29 per hour |
