Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions. We hire people that are skilled and passionate to meet our mission to end homelessness through outcome-based strategies which support personal and community transformation.

The Recuperative Care Program (RCP) is an innovative service that provides immediate access to emergency housing, primary care, and intensive case management support for individuals who have medical support needs while experiencing homelessness. The service partners with Portland area hospitals and clinics, in concert with CCC's patient-centered medical home(s), to help patients receive the right level of care and support for their medical and social needs.

The Recuperative Care Coordinator role is a unique blend of interfacing with clients served by the Recuperative Care Program and overseeing the program's critical funding authorizations and referral management system, working in close partnership with hospital systems and Medicaid payors that support providing this service to their patients and members. This role also collaborates closely with the multi-disciplinary Recuperative Care Program team, which provides intensive case management for patients in a post-hospital recuperative care setting for adults facing homelessness at discharge. The team is focused on working with partner hospitals and referring agencies to provide for the acute and ongoing medical care for program participants, providing short-term supportive housing, crisis management, referral to appropriate substance abuse, physical health, and mental health treatment, access to benefits and entitlements, appropriate and affordable housing, as part of a comprehensive approach to ending homelessness and achieving self-sufficiency.

Location: Evergreen Crossing (8225 NE Wasco St, Portland, OR 97220)

Schedule: Monday - Friday, 8:30am to 5:00pm

Compensation: Staring at $24.52 per hour

MINIMUM QUALIFICATIONS:



  • Education and experience sufficient to succeed, carry out responsibilities, and demonstrate the skills and abilities described above with at least one year of experience working in a pre-hospital or post-hospital setting. Possible experience includes: Medical case management/care coordination, social work, counseling, behavioral health work, other social service work, emergency medical technician work, medical support staff, and nursing.
  • Must have current Healthcare Provider CPR/First Aid certification prior to start.
  • Two years of authorizations management, medical case management, care coordination, or recuperative care/medical respite experience is preferred. Those with relevant lived experience are encouraged to apply.
  • Bachelor's degree in a healthcare-related field preferred.
  • Has the competencies necessary to:

    • Communicate effectively orally and in writing
    • Understand medical related terminology and effectively communicate the medical necessity for services in concert with RCP medical review staff.
    • Understand insurance authorization framework
    • Perform confidently with computer software


  • Must have an understanding of the causes of homelessness, the medical and psychosocial sequelae of homelessness, and the systems and processes used to help clients secure and maintain stable housing.
  • Must have a high tolerance for ambiguity and change.
  • Must have ability to relate to patients who are substance-affected with acute mental health issues and poor hygiene.
  • Must have or quickly develop a working knowledge of medical terminology.
  • Must be able to be trained in the use of Electronic Health Records system.
  • Must be able to occasionally lift items of approximately 10 pounds, stoop, kneel, and climb stairs on a regular basis.
  • Must have non-violent de-escalation training (or complete within 90 days of hire.)
  • If driving, must possess a current driver's license, pass a DMV background check and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training.
  • Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Ace of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.
  • Must have excellent reading and comprehension skills.
  • Must generally have the ability to walk, bend, stoop, kneel, squat, twist, reach, pull and lift up to 50 pounds, and climb stairs with reasonable or no accommodation.
  • Must pass a pre-employment drug screen, TB test, and background check for work at Central City Concern and to obtain ID-Badge at Partner Hospitals. This includes clearance by the DHS Background Check Unit.
  • If in recovery, must be able to document continuous abstinence under independent living conditions or recovery housing for the immediate past three years.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Referrals and Authorizations



  • Work to support facilitation of RCP referral management processes and develop positive working relationships with RCP internal and external referral partners.
  • Represent RCP to funding partners (contracted insurance partners and hospital systems) through both in person and remote meetings and through day-to-day operations as needed.
  • Providing training and orientation to referral partners from a variety of healthcare settings on RCP and on the referral process.
  • Develop knowledge of RCP eligibility criteria, in concert with program leadership.
  • Coordinate directly with and develop relationships with area hospitals, and home health agencies.
  • Document referral management activities and communications in Electronic Health
  • Record with accuracy and timeliness.
  • Gather Assemble and integrate information from medical records from multiple sources and substantial partnership with RCP's interdisciplinary team to understand and document program participants' need for services., including their clinical status and referral needs.
  • Utilize existing systems to track new program referrals, program enrollees, and past program participants to assure accurate data about program referral, enrollment, and authorization status.
  • Understand and exercise service eligibility framework in working to make requests for continuation of services with funders.
  • Maintain working knowledge of RCP participants' status through the course of their care trajectory.
  • Partner with other internal departments to assure the accuracy of RCP's authorizations tracking system.
  • Manage system of authorizations tracking, including qualitative and quantitative client-specific data relevant to funding authorization for RCP services.
  • Furnish information relevant to RCP client funding authorization status to internal and external partners, including CCC's Billing and Business Information Systems department, health services leadership, community partner liaisons in both health services and insurance payer entities.
  • Develop reauthorization requests for RCP clients based on medical and social necessity.
  • Manage authorizations for new referrals, drive processes for reauthorizations over the course of program participants' enrollments.
  • Partner with other internal departments to assure the accuracy of RCP's authorizations tracking system.
  • Follow up on outstanding authorization requests to clarify authorization determinations, and work to appeal determinations when assessed to be appropriate given case-specific circumstances.
  • Work to adjust authorization requests based on changes in RCP participants' clinical status, care trajectory, or placement opportunity.
  • Facilitate Release of Information processes as required.
  • Participate in compliance, quality assurance and quality improvement activities.
  • Attend all mandatory CCC trainings in a timely manner..
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediate reporting of any PHI breach of CCC or outside provider health records to the CCC Legal and Quality departments, as well as to the program administrator.
  • Perform other duties as assigned.


Milieu Support



  • Greet RCP clients and visitors at the RCP Front Desk.
  • Monitor access to the Recuperative Care Program entrance.
  • Respond to emergency medical situations as needed, complete pre-hospital care reports and incident reports when necessary.
  • Provide social contact for active RCP clients and assist clients with accessing RCP resources, including social and/or otherwise therapeutic group opportunities.
  • Direct clients to the appropriate RCP staff person as necessary.
  • Effectively de-escalate heightened situations with patients experiencing trauma, exacerbated mental health symptoms, and behavioral complexity.
  • Actively participate in inter-disciplinary case collaboration, including regular case reviews with RN Complex Care Manager.
  • Exercise Trauma Informed Care and Harm Reduction principles in providing care for RCP participants.
  • Support the implementation of behavioral interventions when appropriate, in conjunction with interdisciplinary team and Clinical Supervisor.
  • Participate in RCP phone management.

  • Ability to develop and maintain positive working relationships with other programs staff, hospital partners and health service providers. Ability to develop and maintain positive working relationships with other community resources critical to program success.
  • Ability to maintain accurate records and necessary paperwork.
  • Ability to learn and apply training instruction.
  • Ability to perform simple arithmetic computations.
  • Ability to handle emergency/violent situations with skill, confidence and tact.
  • Ability to supervise Recuperative Care volunteers when appropriate. Must have a high tolerance for ambiguity and change.
  • Ability to maintain strict standards of confidentiality with regard to patient information.
  • Ability to maintain a calm professional demeanor and make independent judgments.
  • Ability to request and track funding authorizations.
