Now Hiring: Join Central City Concern's fantastic Nursing team as our Overdose Prevention & Aftercare Specialist RN

Location: Central City Concern - Golden West (Downtown Portland; 707 NW Everett Street, Portland 97209)

Schedule: Monday - Friday (8:30am - 5:00pm)

Compensation: $44 - $57/hour (final rate dependent on years of formal RN experience + CCC internal equity review)

MINIMUM QUALIFICATIONS (REQUIRED)



  1. Required: Current Oregon RN License.
  2. Required: Must possess a current driver's license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern's Fleet Safety policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training. Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.



PREFERRED QUALIFICATIONS (GREAT TO HAVE, BUT NOT REQUIRED)



  1. Preferred: Bachelor's of Nursing (BSN).
  2. Preferred: QMHP-R, at minimum, preferred.
  3. Preferred: Experience providing care coordination, case management, or other services to an under-served adult population experiencing mental illness, substance abuse, and/or houselessness.
  4. Preferred: Experience working with dually-diagnosed individuals preferred.



SUMMARY: The Overdose Prevention & Aftercare Specialist, Nurse is part of a multidisciplinary team providing care coordination, compassionate interventions, and support to clients at elevated risk of opioid overdose, leveraging harm reduction principles and trauma-informed, evidence-based practice. Key responsibilities include providing assessment of risk and current service gaps, medical and wellness services including triage and hands on care when resources are available, crisis intervention, outreach for initial and ongoing medical care engagement; facilitating interdisciplinary case conferences, and service engagement through face-to-face outreach and phone contact.



ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Provide same-day and next-day outreach, engagement, and brief solution-focused crisis interventions to CCC clients who have experienced an opioid overdose.
  2. Participate in referral review and prioritization of CCC clients identified as at risk of opioid overdose.
  3. Support the proactive identification of eligible clients through review of incident reporting data, Collective records, medical records, and other resources.
  4. Collect information regarding clients' history through a combination of interviewing, records review, and consultation, after obtaining written consent.
  5. Provide consultation and recommendations to team members, program leaders, and supportive housing care team members regarding medical acuity, chronic medical conditions, and medical sequelae of substance use.
  6. Facilitate interdisciplinary case conferences, including clinical care team members and Housing and Supportive Housing care team members.
  7. Leverage systems knowledge and care pathways to support clients in rapidly accessing withdrawal management, outpatient therapy, medication-assisted treatment, primary care, and other services as indicated.
  8. Participate in outreach activities necessary to offer services and engage participation in a non-coercive manner. Escort clients to meetings and appointments as necessary.
  9. Support clients in accessing and storing medical supplies, including harm reduction supplies and wound care supplies
  10. Provide follow-along monitoring of clients following intervention, and support re-engagement in overdose prevention and other services as appropriate.
  11. Participate in the development of care pathways, access improvement strategies, and other service design activities.
  12. Collaborate with health plans to ensure that clients are able to access available benefits and supports.
  13. Collaborate with residential facilities, subacute facilities, and hospitals to support admission planning, care delivery, and discharge planning. Provide services to clients while admitted to inpatient facilities as needed.
  14. Create and deliver opioid safety and harm reduction training, education, and technical assistance to CCC staff, teams, and leaders across our continuum of services, using a variety of instructional techniques such as didactic lecture, role playing, experiential learning, team exercises, group discussions, and multi-media.
  15. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  16. Provide education to clients regarding health effects of substance use as it relates to their health conditions using rapport and trauma informed approach.
  17. Perform Medication Reconciliations, and adherence monitoring as needed.
  18. Assist in the coordination, delivery and administration of medications. Provide medication education when needed.
  19. provide supportive wound care education, facilitate wound care plan, and refer to specialty wound care as appropriate.
  20. Other duties as assigned.



SKILLS AND ABILITIES:



  1. Ability to consider the impacts and outcomes for underserved communities during decision-making process.
  2. Ability to consider impacts of oppression, structural racism, and individual bias on client outcomes.
  3. Demonstrated commitment to harm reduction principles and techniques, and an inclusive approach that honors and values all pathways that support individual recovery.
  4. Knowledge and skills in behavioral health nursing assessment techniques, including ability to perform complex risk assessments that incorporate medical acuity, management of chronic health conditions, acute withdrawal risk, and overdose risk.
  5. Knowledge of harm reduction strategies, motivational enhancement therapy, motivational interviewing, and other evidence-based practice interventions.
  6. Able to quickly and accurately assess the needs of individual clients, family dynamics and potential barriers to service.
  7. Ability to facilitate interdisciplinary care teams, including knowledge of medical systems and terminology and ability to effectively lead case conferences with a broad array of clinical and non-clinical stakeholders.
  8. Ability to effectively train and coach others.
  9. Advanced communication skills, with the ability to communicate clearly, concisely, and appropriately both orally and in writing
  10. Highly adaptable, with the ability to respond flexibly and positively to change and apply past learning to novel situations.
  11. Ability to work in an environment subject to ambiguity and change.
  12. Ability to learn new skills and have a demonstrated ability to apply principles learned to new situations.
  13. Knowledge of de-escalation methods or ability to be trained in de-escalation methods.
  14. Ability to display a non-judgmental attitude.
  15. Demonstrated knowledge of community and social service agencies.
  16. Ability to perform nursing assessments, triage and coordinate appropriate interventions and plan.
  17. knowledge of health effects of substance use, including mode specific considerations, and substance specific considerations.
  18. knowledge of physiological considerations related to substance intoxication, particularly with respect to considerations around concomitant use, and use concurrent with chronic medical conditions.
