The APRN Supervisor:
- Oversees and supervises the hospice APRNs and ACAs (Administrative Clinical Assistants).
- Coordinates and manages the F2F visit process, ensuring timely coverages for all required F2F visits, in collaboration with the F2F schedulers.
- Coordinates and manages the schedule for First on Call APRNs.
- Drives certification and quality initiatives with team.
- Collaborates with Hospice Medical Director on strategic use of APRN/ACA team.
- Coordinates onboarding and training for APRNs and ACAs.
- Works primarily assisting with Face-to-Face Hospice visits but may provide care in multiple sites that include but are not limited to home, hospice residence, hospice inpatient unit, acute care facility, hospital-based practice, skilled nursing facility and clinics.
- Promotes positive health behaviors and self-care skills through education and counseling.
- Participate in the team responsibility of providing 24 hour/7 days a week patient care.
- Favorably represents Empath Health when speaking with other professionals including physicians, hospitals, skilled nursing facilities, resource agencies, and the medical community.
- Demonstrates knowledge of Empath Health and the partner facility policies and procedures and pertinent State and Federal Rules and Regulations.
- Supports the mission of Empath Health through formal and informal education.
- Participate in continuing professional development and maintenance of clinical skills.
- Acts as a health care resource, hospice consultant, and patient advocate.
- Participates in inter/intra-agency and community referrals.
- Maintains a written, signed protocol of mutual agreement between the APRN and physician(s) that stipulates practice guidelines and is filed annually with the Florida Department of Health.
- Performs other duties as requested by the Hospice Medical Director and/or Chief Medical Officer
Qualifications and Requirements:
- Education: Master's degree in nursing or a related field.
- Licensure: Current Florida license as an Advanced Registered Nurse Practitioner, certified in a practice are (if applicable to departmental requirements). Current DEA (Drug Enforcement Agency) license.
- Certifications: Valid CPR certification from the American Red Cross or American Heart Association.
- NPI provider and ability to bill CMS services or ability to obtain same (if applicable to departmental requirements).
- Current DEA License.
- Minimum of two (2) years of nursing experience required.
- Minimum of two (2) year experience as an APRN required, with experience in adult, gerontology, or oncology practice preferred.
- Experience with supervising clinical staff highly preferred.
- Experience with symptom management and pain management is highly preferred.
- Experience in Hospice or Palliative Care preferred.
- Ability to make home/residence visits
- Strong communication and assessment skills.
- Valid Florida driver's license and automobile insurance as per agency policies
- Basic keyboarding and computer skills including Microsoft Outlook, Word or comparable programs
