The MTM pharmacist assumes a primary responsibility and accountability for the provision of safe and effective drug therapy and other medication management services intended to achieve outcomes related to the mitigation or prevention of disease, reduction of patients' symptoms, prevention of adverse drug events and improvement of patients' quality of life. Required Qualifications:
- Candidate must have graduated from an accredited college of pharmacy on or after May 1996, or completed a structured and comprehensive education program approved by the Accreditation Council of Pharmacy Education (ACPE) and the Board of Pharmacy with didactic and clinical elements upon hire.
- Candidate must have two years of recent clinical pharmacy experience in ambulatory care as part of a multidisciplinary team within the last five years, or successful completion of one year specialized post-doctoral residency.
- Candidates must have a current Wisconsin Pharmacist license (or be eligible) and maintain that license in good standing.
Preferred Qualifications:
- Board Certified Ambulatory Care Pharmacist (BCACP) or Board-Certified Pharmacotherapy Specialist (BCPS) preferred.
- Fulfill credentialing requirements of HSM.
- This position requires credentialing, following the National Committee for Quality Assurance (NCQA) guidelines.
- Strong leadership and organizational skills. Maturity and dependability are essential.
- Ability to organize and function under stress with many interruptions.
- Ability to establish and maintain effective, friendly and courteous working relationships with coworkers, physicians and patients.
- Strong oral and written communication and listening skills.
- Ability to accept and implement change.
