Join our team which is committed to the delivery of the highest quality healthcare service. We are seeking a full-time HIV RN Case Manager & Team Lead who is self-motivated, energetic, and a take-charge individual.

Responsibilities:


  1. Oversee and manage all members of the HIV Case Management team which includes management of time and attendance, conducting performance evaluations, and being involved in disciplinary procedures with the assistance of the Director of Care Coordination Programs.
  2. Schedule, design, and conduct department meetings. This includes organizing and tracking any mandatory training that needs to be completed.
  3. Organize the hiring, training, and onboarding for all new hires.
  4. Conduct chart audits and work with HIV Case Management team to ensure continuity of care for patients by complying with documentation of eligibility and other program requirements in the EMR accordingly.
  5. In coordination with the Director of Care Coordination Programs, lead the development, implementation, and evaluation of current and new projects within the RW program.
  6. Under the guidance of Director of Care Coordination Programs, participate in the preparation of grant applications and reports.
  7. Lead the facilitation of RW Consumer Advisory Board (CAB) to present information about current services and quality projects to RW patients and obtain input to inform the development and implementation processes of projects and services.
  8. Provide and coordinate care and treatment for new and existing SSCHC patients in the HIV program.
  9. Work collaboratively with physicians, behavioral health providers, and the HIV Case Management team to assure the coordination of quality health care for patients with HIV.
  10. Order laboratory studies per HIV protocol and assign the new patient to a physician designated to treat HIV.
  11. Provide direct nursing services to patients, as directed by the physician.
  12. Appropriately document all patient encounter/interactions, including phone contacts. Review patient's Progress Notes in Electronic Medical Record (EMR) for care plan and orders. e.g. in-house or external referrals, labs, x-ray etc. Review tasks in EMR throughout the day for new orders.
  13. Coordinate and facilitate inter-agency communication regarding patient referrals and care plan.
  14. Provide ongoing patient education regarding medication compliance, HIV transmission and prevention, HIV progression, safer sex/safer IV drug use and confidentiality.
  15. Make patient home visits to facilitate compliance with health care plan and medication adherence.
  16. Triage HIV patient phone calls regarding need for clinic visit between quarterly appointments.
  17. Be available for patient office visits by appointment or drop-in.
  18. Refer patients for appropriate off-site services, including scheduling appointments.
  19. Provide back-up to the other HIV Case Managers.
  20. Participate in community HIV conferences and training opportunities.
  21. Attend and participate in Quality Improvement Meetings, HIV Multidisciplinary Meetings, and other applicable meetings.


Qualifications:

  1. Registered Nurse with Wisconsin current license.
  2. Two years of nursing experience desired.
  3. Experience in Community Health, Public Health or a community agency and direct experience to persons with HIV preferred but not required.
  4. Current BLS or ACLS certification.
  5. Ability to relate to people from diverse ethnic and cultural backgrounds.
  6. Valid Wisconsin driver's license, use of an automobile and current automobile insurance.
  7. Bilingual (Spanish/English required).
