Nurse Practitioner - DLD Navigator Program

Position Summary

The Nurse Practitioner (NP) will provide comprehensive clinical support for the Navigator-facilitated direct-to-procedure screening program within the Department of Medicine. Operating under the clinical supervision of the Clinical Director, the NP will play a pivotal role in ensuring patient safety, optimizing procedural preparedness, and facilitating effective communication across the care continuum. This position is integral to the success of the open-access pathway for endoscopic procedures.

Responsibilities

Preoperative Duties:

• Conduct comprehensive preoperative clinical reviews of open access referrals, applying established criteria to confirm procedural indication and patient suitability for the open access pathway.
• Collaborate with consultants and referring providers to coordinate necessary preoperative medical management and address any clinical concerns.
• Develop and provide individualized modifications to bowel preparation instructions, considering comorbid conditions (e.g., chronic anticoagulation, diabetes, Obstructive Sleep Apnea (OSA), morbid obesity).
• Identify and address risk factors for suboptimal bowel preparation (e.g., chronic constipation, opioid use, history of poor preparations) and implement strategies to optimize procedural success.
• Provide clinical supervision and support to the program's Registered Nurse (RN) in delivering patient education regarding prep instructions and addressing preoperative questions and concerns.
• Serve as a key medical liaison to referring providers, ensuring seamless communication and care coordination.
• Offer clinical expertise and support for prior authorization requests as required.

Postoperative Duties:

• Review procedure and pathology reports. Under the guidance of the Program Director and performing endoscopist, communicate results effectively to referring providers and patients through written correspondence, telephone communication, and in-person consultations when clinically indicated.
• Ensure timely and accurate updates to the Health Maintenance module in the Epic electronic health record (EHR) system to reflect follow-up recommendations.
• Facilitate appropriate referrals for patients requiring further consultation or management from other specialists (e.g., Colorectal Surgery, Interventional Gastroenterology, GI Motility services).

General Duties:

• Represent the Navigation Program at external events, meetings, and conferences as designated by the Clinical Director.
• Participate in research activities and clinical protocols pertinent to the Navigation Program and patient outcomes.
• Conduct outreach initiatives to referring clinics and community-based organizations to promote program services and foster collaborative relationships.
• Foster a collaborative working relationship with all members of the multidisciplinary team, including medical leadership, RNs, patient navigators, fellows, and endoscopy unit staff.
• Perform additional related duties as needed.

Minimum Qualifications

• Master's degree or equivalent in education and experience, plus one to two years of experience.
• Current New York State NP and RN licenses, and board certification
• Current Drug Enforcement Administration (DEA) registration.
• Excellent communication (written and verbal), interpersonal, and organizational skills.
• Availability for a full-time, five-day work week schedule.

Preferred Qualifications

• Bilingual proficiency in English and Spanish.
• Prior clinical experience in Gastroenterology, Endoscopy, or General Surgery.
• Proficiency with the Epic electronic health record (EHR) system.

Other Requirements

• Obtain and maintain any additional clinical and research certifications as required by Columbia University Irving Medical Center and New York-Presbyterian Hospital, as determined by the Clinical Director to support role responsibilities.
• Successful completion of applicable compliance and systems training requirements
• Medical clearance conducted by the NYP Workforce Health & Safety Department
