Lead, Nurse Practitioner (IKC)

The Lead, Nurse Practitioner ("NP Lead") functions autonomously, yet in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for ESRD and/or CKD patients. The NP Lead will focus on improving clinical outcomes, care delivery, goal setting, and onboarding new NPs.

As the NP Lead you will perform Comprehensive Health Evaluations during patient visits in dialysis clinics, hospitals, and homes when necessary to provide service to identify and assist with patients' needs including but not limited to medical, social, emotional and financial concerns through medical record review, consultation with patient, family and healthcare team members.

As the lead you will act as the clinical lead for a team of NPs; assist with onboarding new NP's; complete ongoing training for NPs through clinical best practice exercisesand provide ongoing support to other NPs.

Requirements:

• Current license to practice in the state of practice

• Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire

• Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification

• Minimum of two (2) years' experience as a Nurse Practitioner (NP)

• Preferred experience as a Family Nurse Practitioner (NP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology

• Demonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluationof chronic patientsrequired

• Intermediate computer skills and proficiency in MS Word, Excel, and Outlook required

• Must be willing and able to complete home visits as needed
