The Guidance Center (TGC) is the Community Mental Health Center and Certified Behavioral Health Clinic serving Leavenworth, Atchison, and Jefferson counties in Kansas. The Guidance Center (TGC) is currently seeking a Hospital Liaison. This individual provides hospital liaison services to The Guidance Center clients and local hospitals in TGC catchment areas. This individual participates in admission, ongoing treatment and discharge planning, follow-up services to ensure a smooth transition from the hospital placement back to community-based services, communicates with community partners, tracks care and treatment processes and provides updates to TGC providers on clients that are screened or admitted for hospitalization. Provide support as needed to Guiding Lights and Mobile Crisis. Carry a small caseload of clients who were recently released from hospitalization and are pending placement in CSS services.
Coordinate information and services between TGC and state and local hospitals for clients who are screened for inpatient services that relate to a consumer's admission, ongoing treatment, discharge and outpatient follow-up.
Build relationships and improve communication between hospitals and TGC offices.
Provide continuity of care including discharge planning and appointment scheduling for TGC clients who are inpatient and help assist with discharge prior to clients leaving inpatient settings.
Coordinate with county attorney offices for court hearings for clients who are being involuntarily placed, provide notification to QMHP hospital screener of court dates, and provide updates to client's TGC provider.
Develop data tracking for clients who are screened and outcomes associated with screen. This also includes tracking client's admission, length of stay, engagement of TGC services post discharge and re-admissions.
Maintain a small caseload of clients providing bridge case management services from when clients are discharged from the hospital until they can be enrolled in CSS services.
Gather information about the client and refer to the appropriate resource to handle the problem such as a same day appointment with a licensed therapist, emergency room at local hospital, same day appointment at local office, welfare checks, diagnostic evaluation, etc.
Provide support to other providers both in and out of the office, as well as to other community entities dealing with a client in crisis and needing the assistance of crisis stabilization services.
Make follow-up calls to clients who fail to show for post crisis follow-up appointments or who may be needing extra contact to effectively cope with a current Crisis.
After a QMHP has completed an evaluation, assist in coordinating access to the appropriate level of care.
EDUCATION and EXPERIENCE:
Bachelor's degree is preferred.
Equivalent work experience in the human services field, or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education, may be considered
One year volunteer or paid experience working with individuals in the community.
