Responsibilities

We are seeking a talented person to join our growing team as our Intensive Outpatient Program (IOP) and Partial Hospitalization Program (PHP) Care Navigator. This role is responsible for a person-centered patient experience throughout the IOP and PHP process from pre-admission to post-discharge.

St. Louis Behavioral Medicine Institute (SLBMI) is home to several specialty IOP and PHP programs, including the Center for OCD and Anxiety-Related Disorders, Dialectical Behavior Therapy Program, Eating Disorders Program, JumpSTART (General) Program, and Radically-Open Dialectical Behavior Therapy Program. The Care Navigator works closely with our Clinical Intake Specialists as well as our Directors of the aforementioned programs to collaboratively assist patients with a seamless entry into our higher level of care.

Essential job duties include:

  • Meet with patients (and guardians as applicable) in an individual and/or group format prior to admitting into their identified IOP/PHP program, with the goals of completing paperwork and outcome measures and orientation to the program.
  • Request prior authorization from insurance companies and maintain ongoing authorizations as needed throughout the IOP/PHP Program.
  • Act as the ongoing liasion for patients (and guardians as applicable) while they are enrolled in the program and communicate actively with the IOP/PHP treatment teams for patient needs or concerns.
  • Coordinate outpatient care, to include facilitating internal referrals for other services while the patient is in IOP/PHP, discharge planning assistance post-IOP/PHP program, and communicating with the patient's external providers (as applicable and with proper release) at admission, mid-treatment, and at discharge.
  • Ongoing performance improvement monitoring, including data tracking and reporting of admission timelines, symptom surveys, and pateint satisfaction surveys.

This position is located onsite at the Macklind location 3 days per week and onsite at the Chesterfield location 2 days per week.

Qualifications

Qualifications:

  • Post-secondary education in social services, health-care or related field, or an equivalent combination of work experience and education
  • Familiar with office machinery (Fax, printer, scanner, etc) with minimum intermediate knowledge of Microsoft Office Suite products; must be able to navigate in Excel. Prior Electronic Medical Record experience is a plus.
