Medica's Clinical Care Team Supervisor is responsible for managing a team of care coordinators for the provision of care coordination services for State Public Programs and other Medica members. The majority of members will be from our Care Coordination Products (MSHO, MSC+, ISNBC and SNBC) within State Public Programs and the Government division. This opportunity will support our Twin Cities Metro area, with some being in the Northern part of MN, and will provide direct supervision to the care coordinators as well as direct oversight for the assigned panel of members, a subset of a larger panel of members for the geographic region of the products' service areas. In addition, this leader will partner with our Clinical Care Manager in the implementation and evaluation of Medica Care System care coordination services, implement improvement opportunities and manage corrective active plans as needed. The employee may need to occasionally come onsite to the Minnetonka office for applicable heads-up work. Frequency is determined by business need as decided by leadership. The full salary range for this position is $68,800 - $118,000. Annual salary range placement will depend on a variety of factors including, but not limited to, education, work experience, applicable certifications and/or licensure, the position's scope and responsibility, internal pay equity and external market salary data.
