As a Community Health Worker (CHW), you will serve as a liaison for Medicaid members, helping them access appropriate care, navigate community resources, and manage their health through education, screenings, and care coordination. The CHW also addresses Social Determinants of Health needs such as transportation, access to care, housing, and food insecurity. This position focuses on improving health outcomes by fostering strong relationships with members, promoting preventative care, and ensuring seamless transitions across healthcare settings.

This is a field-based position with a home-based office, meaning you will work from home when not in the field. Field-based work may include traveling to an alternate hospital or clinic setting to complete work duties based on health plan needs. The standard schedule is Monday through Friday, between 8:00 AM and 5:00 PM. After six months of employment, established staff may be eligible for an alternative schedule, working four 10-hour days (7:00 AM to 6:00 PM) instead of the traditional five-day workweek.

Travel 50% in Greensboro, NC and surrounding areas within 75 miles.

Primary Responsibilities:



  • Conduct telephonic and in-person outreach to engage members, especially high-risk or hard-to-reach individuals
  • Build positive, trusting relationships with members to encourage active participation in their care
  • Help members set person-centered SMART goals and develop care plans with regular follow-up and documentation
  • Use motivational interviewing techniques to empower members in managing their health
  • Support transitions of care and help members stay compliant with care plans
  • Ensure members attend regular primary care visits and assist with prescription pick-up
  • Collaborate with care teams, including providers, internal staff, and community partners, to ensure coordinated, high-quality care
  • Complete delegated tasks from the care team and communicate relevant health information to all stakeholders
  • Conduct individual-level screenings such as Health Risk Assessments (HRAs) and Transitions of Care assessments
  • Identify members' healthcare needs, preferences, and goals to inform care planning
  • Assess, plan, and implement customized care strategies aimed at the least restrictive level of care
  • Link members to appropriate community and company resources, including social services, financial aid, and psychosocial support
  • Participate in community events related to preventive health and wellness
  • Maintain accurate and timely documentation of member interactions, care plans, assessments, and referrals
  • Adhere to detailed documentation standards and privacy regulations
  • Demonstrate cultural competence and a commitment to learning about community values and needs
  • Exhibit strong organizational, time management, and multitasking skills
  • Be adaptable, patient, and self-motivated in a dynamic work environment
  • Perform other duties as assigned

Required Qualifications:



  • High School Diploma/GED (or higher) OR 5+ years of equivalent case management experience
  • 2+ years of experience in a field-based community health role
  • 2+ years of experience in familiarity with local community resources, culture, and values
  • Intermediate level of computer proficiency, including Microsoft Outlook and Teams, and ability to use multiple web applications
  • Must be a North Carolina resident and have lived in the local community for at least 2 years
  • Must reside within Guilford County, North Carolina
  • Ability to travel locally up to 50% of the time, with trips up to 150 miles round trip (mileage reimbursement provided)
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information and live in a location with high-speed internet capabilities
  • Access to reliable transportation & valid US driver's license


Preferred Qualifications:



  • Bachelor's degree (or higher) in Social Work, Health Care Administration, or a related field
  • Licensed Practical Nurse (LPN), Certified Nursing Assistant (CNA), Home Health Aide, or Medical Assistant certification
  • Community Health Worker (CHW) accreditation
  • Experience working in managed care settings
  • Knowledge of Medicaid and/or Medicare populations
  • Experience with maternity and pediatric populations
  • Home health or field-based experience, including home visits
  • Experience working in team-based care environments
  • Ability to solve routine problems independently and collaborate with supervisors on more complex issues
  • Strong organizational skills with the ability to prioritize tasks and meet deadlines
  • Strong communication and customer service skills both in person and via phone
  • Ability to work independently and maintain good judgment and accountability
  • Demonstrated ability to work well with others
  • Strong organizational and time management skills
  • Ability to multi-task and prioritize tasks to meet all deadlines
  • Ability to work well under pressure in a fast-paced environment
  • Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $20.00 to $35.72 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
