Position Summary |
The Project Directors from the five-year
HRSA funded grant entitled Transition for NC Youth with EpilepsyPlus:
UNC Whole Brain Health Program (
WBHP), are seeking a qualified candidate for a part-time Clinical Coordinator position. The
WBH Clinical Coordinator will work closely with the PI/Co-PI, an interdisciplinary team of
CYSHCN practitioners/clinicians/translational researchers, multi-sectoral partners, and key state and national stakeholders to center the needs of youth/young adults (14-26) and their families/caregivers receiving care through
UNC Pediatric and Adult Neurology. The Clinical Coordinator will be a nurse practitioner with extensive clinical experience in Pediatric Neurology, Epilepsy, Intellectual and Developmental Disabilities (
IDD) and supporting adolescents with complex healthcare needs as they transition to adulthood. They will be responsible for Transition Clinic referral coordination, clinic flow, and day-to-day operation of protocols and evidence-based practices. They will serve as the patient's initial contact with the
WBHP /
UNC Neurology Transition to Adulthood Clinical team by providing outreach prior to their initial clinic visit and by administering and reviewing forms including standardized transition to adulthood readiness assessment tools, mental health questionnaires, and measures that assess quality of life and caregiver stress. Additional tasks include working with others on the
WBHP team to develop educational curricula for interdisciplinary trainees and precepting
UNC nursing students. This position will work a hybrid schedule with flexibility to seek complementary funding support. Depending on the training and qualifications of candidates, there may be additional support opportunities identified through related projects. |
Minimum Education and Experience Requirements |
Bachelor's degree in a discipline related to the field assigned and one year of related training or experience; or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions. |
