In this role, you will support the Regional Population Health Improvement Manager (PHIM) and the Population Health Quality Team by performing administrative and documentation tasks essential to achieving Population Health, Value-Based Care, and Quality objectives.Your responsibilities will include patient outreach and engagement, pre-visit planning, data tracking in payer portals, and other administrative processes to meet value-based requirements. You will also contribute to clinic operations through panel management, data collection, documentation for quality initiatives, and care gap closure. Additionally, you will assist in implementing standard work and best practices to enhance care delivery.
