Job Summary

The Streets to Home (STH) Intensive Case Manager position on the Horizon House housing team exists to provide comprehensive case management and housing support services to individuals or families entering permanent supportive housing after experiencing homelessness. Services are provided utilizing a Strengths-Based, Harm Reduction approach within a Housing First model. The position will effectively implement problem-solving skills specific to socioeconomic, psychosocial, and environmental issues to empower individuals to participate and contribute fully as members of the community. Expectations include completion of thorough assessments; identification of client resources, talents, skills, and barriers; and collaboration with clients to identify goals and develop actionable plans. Housing stability is a primary goal of Horizon House housing services.

Streets to Home Indy is a citywide initiative led by the Coalition for Homelessness Intervention and Prevention (CHIP) in partnership with the City of Indianapolis, service providers, and community leaders. The program aims to reduce and ultimately end unsheltered homelessness by 2028 through a Housing First approach that combines rent assistance with intensive support services. Case management is central to the initiative, ensuring that individuals have the stability and support needed to remain housed long term. This position reports to the Program Manager.

Example Duties and Activities

  • Provide comprehensive case management and housing support, including assessment of client strengths, needs, and barriers; crisis intervention; advocacy; and connection to community resources such as medical, mental health, and addiction services.

  • Assess and enroll eligible clients in the CES Housing Support project and collaborate with clients to develop, implement, and modify individualized housing plans aimed at long-term stability.

  • Maintain appropriate and timely documentation, including but not limited to client identifying information, service records, and case notes.

  • Documentation of encounters with clients, as well as services performed on behalf of clients, will be compliant with agency and Indianapolis Continuum of Care standards and protocols and will be entered into the HMIS adopted by the Continuum and utilized by Horizon House.

  • Collaborate with community providers, multidisciplinary teams, and internal staff to coordinate services, advance client progress, address challenges, and ensure access to resources supporting recovery, medical care, mental health care, and other resources specific to client needs and goals.

  • Participate in meetings, trainings, and professional development opportunities, providing mentorship and peer support to team members as appropriate.

  • Contribute to program evaluation, research initiatives, and development activities, including policy, advocacy, funding, and process improvements, as assigned.

  • Assist with client conflicts or other disturbances, intake, and outreach efforts within the agency and community

  • Provide ongoing support and advocacy with the goal of ensuring housing sustainability.

  • Observe policies related to client confidentiality with respect to reports of abuse or neglect or other reports to law enforcement entities.

Required Competencies

  • Degree and Credential Requirements - Bachelor's degree in Social Work, Human Services, or a related field.

  • Industry Expertise - Experience engaging with various populations, including individuals experiencing homelessness or housing instability, or experience with creating plans that have included employment, housing, and/or financial goals that have improved their situation.

  • Building Collaborative Relationships - Ability to engage and establish positive rapport with a diverse group of individuals experiencing homelessness who are on the street or in other locations not intended for human habitation. Skilled in building collaborative relationships across sectors and navigating complex systems of care. Develops, maintains, and strengthens partnerships at all levels with others inside or outside the organization who can provide information, assistance, and support. Is able to receive as well as provide feedback to improve performance or processes; builds and maintains customer satisfaction.

  • Information Monitoring/Reporting - Collects, analyzes, and applies program data to inform strategies and improve services. Experience in data reporting systems is essential. Strong skills in data interpretation, program evaluation, and outcome reporting. Capable of using data to assess impact, identify areas for improvement, and ensure accountability.

  • Communication Skills - Articulates thoughts and expresses ideas effectively using oral, written, visual, and non-verbal communication skills, as well as listening skills to comprehend customers' needs. Delivers information in person, in writing, and in a digital world.

  • Organizational Aptitude - Plans and attends to details and pursues quality in accomplishing multiple tasks in an organized and timely fashion. Exhibits careful attention to detail and follow-through.

  • Technical Knowledge - Has solid knowledge of G Suite, MS Office, databases, the internet, and case management systems.

Other Requirements

  • Background Screening - All candidates must pass a federal background check. Must have a valid driver's license, up-to-date auto insurance, and a good driving record.

  • Ability to occasionally work a flexible schedule beyond normal business hours.
