Job Description: Central State Hospital is seeking a full-time Case Manager. This position will primarily work Monday - Friday, 8 am-4:30 pm. Job Functions:
Performs case management services including evaluation of assigned patients. Builds rapport with patients and families. Provides crisis interventions, interagency conferences, agency/collateral contacts, pre-discharge visits and discharge planning. Maintains and completes service documentation such as psychosocial assessments, case notes, case reviews, court reports, letters, etc., according to prescribed standards. Conducts case reviews and determines current and future treatment and treatment planning, including after-care plans for clients completing treatment. Participating as an inter-disciplinary team member on the unit as it relates to treatment planning, treatment reviews and discharge planning. Initiates 202A petitions. Assesses also for Guardianship and facilitates this process as needed. Carries out policies and procedures related to duty to warn, APS, CPS, Vine notification and any other applicable legal issues. Writes correspondence on behalf of clients and their families such as court reports, letters of recommendation, etc. Conducts case presentations and consultations with other agencies regarding programs and services offered. Completes hospital and departmental required trainings, participates in performance and quality improvement review activities, and attends workshops/seminars to promote professional growth and expertise.

A Master's degree in social work or a related field, and two years of experience, OR a Bachelor's degree in social work or a related field, and must be enrolled in a program to pursue a Master's in Social Work or related field. Experience serving, and basic knowledge of the mental and physical characteristics of persons with serious mental illness. Good communication and interpersonal skills. Ability to manage case load with rapid patient turnover.
