Job Description

POSITION SUMMARY: The Recovery Coach/Community Integration Specialist assists individuals in transitioning into the community via participation in a variety of community activities, enhances participants' life experiences and provides support through development and implementation of community integration projects.

DUTIES AND RESPONSIBILITIES:

  • Develop and provide activities to best promote participant's interest in community integration by tasks including but not limited to providing individual and group psychiatric rehabilitation interventions targeting community integration
  • Assume a lead role in the team development and review of participant's Recovery Action Plan by collaborating with assigned participants to develop their plan
  • Network for, seek out, and develop appropriate community integration opportunities that match participant's interests and skills by researching and participating in community events
  • Provide counseling and supports to the participant to promote interpersonal and soft skills success in the community
  • Plan for and provide participant transportation and travel training; transport participant in agency vehicles as needed
  • Identify and develop home and natural supports for participants served by providing individual and group psychiatric rehabilitation interventions targeting the development of natural supports
  • Develop and implement a plan for coach fading, incorporating natural supports, being sensitive to risks
  • Maintain clear, active, and constructive communication with the participant while facilitating communication amongst the participants, his/her home supports, family and friends, team members, funding source and monitoring entity representatives, supervisor and co-workers with the goal of enhancing participants' recovery
  • Encourage participation in other services of New Visions (e.g., therapy, medication management) as deemed necessary and appropriate
  • Facilitate orientation of new participant to New Visions and recovery by reviewing program protocols and introducing new participants to existing staff and participants
  • Maintain flexible work schedule and availability, including evening and weekend hours, in order to meet participant's needs and schedules. Some on-call responsibilities are required
  • Prepare case records, reports, notes, data, etc. in line with participant's desires, service expectations and program requirements
  • Provide an active contribution to the conception and attainment of program or service enhancements and contribute to the aims and ambitions of the agency
  • Pursue and complete staff training and development opportunities that serve to ensure service quality and outcomes
  • Encourage peer leadership by involving participants in decision-making roles
  • Compile and render participants' data, outcomes and other internal/external reports for funder and licensing requirements
  • Serve as Elwyn liaison to community agencies, businesses, civic organizations, religious organizations, etc. by researching and outreaching local community entities
  • Coordinates community integration service with participants and team members by providing individual and group psychiatric rehabilitation interventions targeting community integration
  • Provide safe starting point for participant not currently engaged in recovery by outreaching participants who are not engaged in order to encourage participation and help problem-solve barriers to participation
  • Perform cardiopulmonary resuscitation (CPR), first aid, and crisis intervention using agency-trained protocols
  • Perform other duties as assigned

IMMEDIATE SUPERVISOR: Clinical Director, Outpatient and Recovery Services

DIRECT REPORTS: None

CONTACTS: Elwyn staff, Recovery team, funding and regulatory agency personnel, families, outside health care providers, Base Service Unit personnel, various community and civic agencies.

EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:

  • Associates degree in human services or related field, Bachelor's degree preferred
  • Two (2) years' experience in mental health preferred
  • Community integration experience helpful
  • Demonstrated ability to work effectively as part of a team
  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
  • Demonstrated strong attention to detail
  • Demonstrated strong time management and organizational skills
  • Must be able to perform CPR and crisis intervention using agency-trained protocols
  • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
  • Demonstrated basic experience with Microsoft Office applications, including Word, Excel, and Outlook
  • Experience using Electronic Health Record (EHR) / Electronic Medical Record (EMR) preferred
