The Clinical Manager is individually responsible to plan, organize and direct the Lifespark services while insuring compliance with regulatory requirements and Lifespark best practices including but not limited to starts of care, care plan development and ongoing support, client satisfaction, caregiver supervision, and engagement and on-boarding of new nurses and caregivers.
Position: Clinical Manager - for Community Home Health
Salary: $92,000 - $100,000/annually, depending on experience
Service Area: Northwest Metro (coverage area: Buffalo, Champlin, Loretto, Anoka, Osseo, Rogers, Elk River, Rogers, Ramsey, Maple Grove, Zimmerman, Albertville, Bethel & surrounding). Plus, mileage reimbursement of 72.5 cents per mile.
Schedule: 1.0 FTE, Full - Time / Monday - Friday, 8:00am-5:00pm (on-call and weekend rotation: every 4-5th, holiday rotation: 1-2 per year)

Position Functions & Responsibilities:
- Complete initial start of care documentation and process. Provides a complete physical assessment and history of current and previous illness(es).
- Regularly re-evaluates client nursing needs.
- Initiates the plan of care and makes necessary revisions as client status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the client and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct patient care as defined in the State Nurse Practice Act.
- Counsels the client and family in meeting nursing and related needs.
- Provides health care instructions to the client as appropriate per assessment and plan of care.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.
- Acts as Case Manager and assumes responsibility to coordinate client care for assigned caseload.
- Supervisory responsibilities for the RN Case Managers with focus on coaching, accountability, culture, and results.
- Make hiring decisions, on-board new employees, and provide ongoing training for caregiving staff.
- Participate in caregiver orientation and completes competency testing/training of caregivers
- Prospectively reviews Client EMR for assessments to ensure appropriateness, completeness, and compliance with state regulations and organization policy.
- Consults with appropriate clinical staff to clarify any data integrity issues and works with clinician to make appropriate corrections per organization policy.
- Notifies organization leadership of problematic trends because of audit reviews.
- Works with RN Case Managers to address trends that affect the agency's adherence to regulations and policies, as well as outcome and process measures noted during review.
- Participates in Quality Improvement and Corporate Compliance activities as assigned.
- Assists with other chart audit activities as assigned.
- Attends educational workshops and reviews professional publications, as needed.
- Timely completion of all documentation as it relates to the care plan
- Understands and complies with all the rules and regulations that govern the responsibilities of a registered nurse
- Completes orientation and annual training requirements as assigned
- Participates in Lifespark's Event Management process including initiating reports, escalation of events and investigations
- Promotes safety in the workplace through real time coaching and participation in safety awareness training
- Maintains on-call and weekend nursing schedule
- Organizes travel efficiently and submits accurate auto use logs by established deadlines
- Tracks all billable activities
- Assists in ensuring processes to monitor and evaluate safety, risk management and infection control programs.
- Monitors compliance with regulatory standards including verification of billed services.
- Acts as a resource to personnel.
- Provides assistance to community education team in meeting community education needs.
- Promotes self-growth and collegial relationships with others in the home care industry.
- Participates in on-call and weekend rotation
- Identify and communicate opportunities where there may be needs for other Lifespark services including home health, hospice, and senior living.
Qualifications:
- Registered Nurse, BSN preferred
- Prefer advanced certification as a Care Manager, Holistic Nursing, Geriatric CM, and/or Gerontology
- 3 years of RN experience in community-based services (assisted living, home care, hospice, public health)
- 1 year of experience serving in a leadership role including regulatory compliance, personnel management and financial responsibility preferred
- 3 years of experience working with seniors
- Experience in customer service/direct member contact field preferred
- Experience supervising caregivers
- Experience using technology including: Microsoft Office, Smart Phone, Calendaring, Clinical Software platforms (EMR), Wi-Fi, Printers, and Internet Browsing
