At Horizon Home Care & Hospice, hospice nursing is more than a job. It's an opportunity to walk alongside patients and families during one of life's most meaningful times, providing comfort, guidance, and compassionate care when it matters most. As a Hospice Case Manager, also known as a Patient Care Coordinator, you'll build trusted relationships, coordinate care across an interdisciplinary team, and help patients remain comfortable in the place they call home.

We support our hospice nurses with a collaborative team, strong clinical resources, flexibility, and a culture rooted in kindness, respect, and service. If you're looking for a role where your nursing judgment, compassion, and ability to connect with people truly make a difference, we'd love to have you join our team.

About Horizon

To some people, home is a place. To others, it's a feeling.

To us, it's whatever our patients need it to be, wherever they are. Horizon has been a trusted presence in the Milwaukee community since 1990. We're a nonprofit organization with 400 employees and 150+ volunteers, proudly serving more than 2,500 patients and families across Southeastern Wisconsin. We're the second-largest full-service home care and hospice agency in the state. Our services span the full continuum of care - hospice, home care, palliative care, elder care, Meals on Wheels, and grief support - so our teams get to do meaningful, wide-ranging work every single day. We've been recognized as a Top Workplace nine times, not because we nominated ourselves, but because our people said so. That's something we're genuinely proud of. Our culture is built on five values: Accountability, Integrity, Service Excellence, Innovation, and Collaboration. They're not just words on a wall - they show up in the way we treat patients, support our staff, and show up for the communities we serve. If you value kindness, real teamwork, and work that leaves you feeling like it mattered, you'll fit right in here.

Monday thru Friday 8-430, 2 holidays a year.
