Responsibilities The PrairieCare Inpatient Social Worker (LGSW required/LICSW preferred) carries a patient caseload at the 101 bed Psychiatric Hospital in Brooklyn Park, Minnesota, serving ages primarily 5-18 and adults 18-35. The Social Worker focuses on facilitating groups, treatment planning, discharge planning, care coordination, communication with caretakers, community providers, and others involved in the care of patients. Social Workers collaborate daily with the Child & Adolescent Psychiatrists and Therapists on the patient's primary Treatment Team to advance the plan of care within the average patient length of stay, 7-10 days.
