JOB SUMMARY: Under supervision of the Director of Population Health, the Clinical Program Manager provides leadership of all programmatic, personnel, and operational activities for specified programs implemented within the Population Health Department. The Clinical Program Manager will design, implement, monitor, and evaluate key program processes and outcomes to improve care coordination across the continuum of services for a defined population of patients with complex medical/psychosocial issues and barriers accessing and navigating related care. The position will incorporate the role of organizational program focal point, staff leader, educator, and client advocate, to define and meet quality metrics and expected program deliverables in a timely manner. The Clinical Program Manager supervises key program staff and is responsible for maintaining coordinated communications with providers and leadership at CBHC, as well as external stakeholders. Analytical and Critical Thinking * Develop program's population-based management strategies and related processes that support patients to manage their healthcare needs and ensure quality care, cost-effectiveness, and patient engagement, based on in-depth understanding of community strengths, barriers, and needs. * Delegate and oversee the care management of patients targeted by program. * Gather and analyze program-related criteria, data points, and guidelines. * Monitor that appropriate documentation is evident in the electronic medical records. * Utilize systems to build and generate reports for analysis and communication. * Maintain mandatory/regulatory reports on progress and management of program Communication * Communicate regularly with program team members, clinic site staff, patients, and the Director of Population Health to address issues and needs. * Serve as contact point, advocate, and source of information about the specified program for staff and community members. Provide significant information of program development, changes and progress to all direct clinical team members. * Represent organization's population health interests related to the program with external parties. Problem Solving and Decision Making * Initiate and delegate care management processes in a quality focused, cost-effective manner across the continuum of care. Patient Focus * Serve as a patient advocate and clinical expert for staff related to care coordination. * Serve as a mentor to clinical teams to aid with establishing collaborative partnerships with patients to assist them in examining patterns of health care needs, decisions, lifestyle choices, and utilization of resources that affect their health. * Coordinate community resources with emphasis on medical, behavioral and social services. * Manage a care management panel within the program. Teamwork * Demonstrate leadership, mentorship and teamwork within program team and also with clinicians, chronic disease care coordinators, RNs, medical assistants, pharmacists, social workers, behavioral therapist, and others as needed. * Facilitate communication and coordination between members of the health care delivery team, involving the client in the decision-making process to minimize fragmentation in services. * Identify opportunities to assist clinical RN case managers to refer and link patients to needed internal Chase Brexton resources such as behavioral health, pharmacy, social work, and dental. * Work collaboratively with clinical and administrative leadership to design and implement program-related protocols, and coordinate with RNs to delegate care management work, as relevant to specified program, consistently across Chase Brexton. Compliance, Policy, and Procedure * Participates in the development of policies and procedures in support of the specified program. * Ensures compliance with all state and federal regulations and guidelines in day-to-day activities. * Provides oversight for all clients engaged with program, per departmental standards. * Report critical incidents and information regarding quality of care issues. Willingness to Learn * Participates in continuing professional growth through attendance at workshops and professional in-services, through individual research and reading, to include communication skills * Performs other duties as assigned. EDUCATION:
SKILLS/ABILITIES * Demonstrated skills in program management, leadership, supervision, advocacy, communication, education, counseling and collaborative work. * Skill in prioritizing the demands of daily operations and organizing resources. * Motivational Interviewing * Excellent verbal communication and written documentation skills. * Ability to effectively utilize and leverage technology systems. * Critical thinking skills to analyze and solve problems * Proficiency in Microsoft Office including Outlook, Word, Excel and PowerPoint; knowledge of or the ability to learn EMR software and other software in order to perform job duties * Demonstrate critical thinking skills when utilizing the nursing process, based on research, evidence-based outcomes and standards of practice to meet patient's health care needs. EXPERIENCE: * Previous experience leading the implementation of public health programs required; pilot phase experience a plus. * Comfortable in staff supervision, delegation, and supportive team management. * Care coordination experience and/or experience working in an outpatient ambulatory, home-based or other community-based setting. * Experienced in motivational interviewing and behavior change coaching to support patients to define and achieve their own health and wellness goals. Education- Required: Baccalaureate degree in nursing Preferred: Masters degree in Nursing, allied health or healthcare management, or public health |
