Job Overview: The Community Health Coordinator will support departmental personnel to plan, develop, organize and manage projects within the department. This includes monitoring daily tasks and communication, as well as creating reports, presentations, and updates for members of management. The coordinator will establish, maintain and monitor a system for tracking project activities, progress and outcomes. The coordinator will support team members and their project s communications, deadlines, meeting schedules, work plans, and related activities. Qualifications Required:
- Bachelor s Degree required, preferably in Public Health or related field.
- Advanced proficiency in Microsoft Office applications (Word, Excel, PowerPoint and Outlook) including report template building, v-look up, pivot tables, data linking, etc.
- Strong proficiency in data analysis tools and techniques such as SQL and Excel.
- Strong problem-solving skills.
- Must have excellent interpersonal, oral, and written communication skills.
- Must be capable of critical thinking and analysis and written conveyance of same.
- Must have excellent organizational and time management skills.
- Excellent presentation skills to communicate the System s health services to the community and to various advisory boards and state-related agencies
- Must be proactive with the ability to recognize potential project impediments
- Strong analytical and leadership skills
- Must have the ability to work within a diverse team as well as independently as required and maintain good relations throughout encounters in the hospital with other department, patients, visitors, contractors, and vendors
- Demonstrates time management skills, resulting in the ability to manage multiple client relationships and prioritize time and resources accordingly
- Travel, as needed, to attend project meetings or provide oversight and assistance for off-site projects throughout the system
Preferred:
- Minimum of two years experience in data analysis and / or project management in a healthcare environment, preferred
Essential Functions:
- Maintain records related to department activities.
- Collects, correlates, analyzes, interprets, and inputs data.
- Maintains and manages the reports for the hospital in accordance with all policies.
- Ensures that appropriate standards of practice follow corporate compliance guidelines, and that results of accreditation surveys and licensure reviews by NJSDOH and JCAHO are satisfactory.
- Prepares reports, statistical analysis, and presentations for the department and / or project.
- Participates in professional organizations at the local, state or national levels pertaining to data management and systems review.
- Maintains all medical records according to proper policy and procedure. Maintains confidentiality and security of patient information and the integrity of statistics.
- Preparing and distributing daily, weekly and monthly reports to distributions.
- Develop and maintain analytics that provide insights on key metrics and trends
- Develop Excel report templates with the associated formulas to automate data and produce the data needed to effectively manage workflows
- Collaborate with key business leaders as well as with the vendors to analyze data and metrics for trends and patterns to help inform business leaders and assist with strategic decisions
- Mine data to arrive at specific and crucial information for the organization ensuring data is presented accurately and timely and sets the context for meaningful discussions and decision making
- Extract and analyze data to drive towards predictive analytics and leading indicators
- Work with EPIC to develop reports as needed
- Prepare PowerPoint presentations as needed
- Ensure the completeness, accuracy and integrity of all data.

- Create long- and short-term plans, including setting targets for milestones and adhering to deadlines
- Delegate tasks on the project to employees best positioned to complete them
- Make effective recommendations when presented with multiple options for how to progress with the project
- Serve as a point of contact for teams when multiple units are assigned to the same project to ensure team actions remain in synergy
- Communicate with key stakeholders to keep the project aligned with their goals
- Perform quality control on the project throughout development to maintain the standards expected
- Adjust schedules and targets on the project as needs or financing for the project change to meet budget targets
